Missing Child Intake Form

This is the MissingKids.ca initial intake form. Filling out this form is the first step in requesting service from MissingKids.ca. A MissingKids.ca caseworker will receive this information and will contact you during regular business hours to complete the intake process.

You can also reach MissingKids.ca at 1-866-KID-TIPS (543-8477). If this is an emergency, please contact the police.

This form is not a replacement for reporting your child missing to police.

If you are currently residing in the province of Quebec, you may also wish to reach out to the Missing Children’s Network. If you are currently residing in the United States, we recommend you contact the National Center for Missing and Exploited Children for assistance.
